I have an issue with the extension OT Testimonials: when I add and send a review, the server returns error 500! can someone help me?

Thank you for your reply, my problem is this: I have installed your component ot-testimonial, and I am quite sure I set it correctly, but when I try adding a review from the front end of the site the server responds error 500.
